We can provide a certificate only to those authorized to have it. Authorized persons include:

the individual named in the record
parents or legal guardians of an individual under the age of 18 (if the parent is named on the individual’s birth certificate or other legal documents)
legally authorized representatives acting on behalf of an individual
next-of-kin, executor or estate administrator of a deceased individual
Catholic parishes or agencies such as Catholic Children’s Aid Society
